Records & Registration at Jamestown Community College manages academic records, such as course registration, enrollment verification, transcripts, and graduation.

Withdrawal forms:

  • Individual Course Withdrawal Form
    • Use when a student withdraws from one or more courses but remains enrolled in at least one course.
    • Each course requires its own Individual course withdrawal form.
  • Total Withdrawal Process
    • Use when a student withdraws from all courses in the semester.
    • If a student is enrolled in only one course, withdrawing from that course is considered a total withdrawal.

Registration forms can be mailed via US Postal mail to: Jamestown Community College - Records & Registration, PO Box 20, Jamestown, NY 14702-0020. We will also accept email in lieu of individual withdrawals, total withdrawals, instructor permission cards, and audit forms, as long as the appropriate permissions are also received.
