Federal Loan Request Forms

• Direct Stafford Loan - If you wish to accept a loan that was offered to you in your 2013-14 JCC Financial Aid Award Notification letter, you can print the loan acceptance form via your Banner account at banner.sunyjcc.edu. After you sign in, click on the Financial Aid tab and select ‘2. Your Eligibility Requirements’. Select the Aid Year 2013-14 and click the Direct Stafford Loan Form link. If you do not have the link, please refer to your 2013-14 JCC Financial Aid Award Notification letter for further information.
• Direct Stafford Loan Supplemental Review Form - complete only if requested by JCC Financial Aid Office
• Parent PLUS Loan - A parent borrower can complete the Parent PLUS loan application process online at studentloans.gov. The parent should sign in using his/her account.

RFI - Request for Information Forms      **Not all RFI forms are required for each student. Only print, complete, and submit a form if the JCC Financial Aid Office has specifically instructed you to do so. View your requirements on Banner (banner.sunyjcc.edu). You can also print your forms directly from Banner.**
